What are the responsibilities and job description for the Clinical Dietition position at Mt. Ascutney Hospital and Health Center?
Position Summary:
Evaluates, Assesses, and plans nutrition care to meet patient nutritional needs:
1. Assesses patient's nutritional status using documented medical information and established nutritional standards
2. Counsels patients and/or families by identifying nutrition-related problems. Develops plans of instruction and nutritional care, and monitoring results.
3. Identifies inpatient at nutritional risk and documents recommendations.
4. Diagnose and develop intervention for patiens with malnutrition
5. Participates in physicians' rounds, discharge planning rounds, and conferences with nutritional implications.
6. Conducts education programs for patients, community groups, and staff.
7. Precept and mentor new staff or existing staff in clinical specialty areas.
8. Develop therapeutic relationships with multiple subspecialty care team members and providers that enables planning and participating across a broader spectrum patient care.
9. Cross-Covers Broader spectrum of patient care areas and subspecialties as needed.
10. Performs other duties required or assigned.
Qualifications:
A Bachelor's degree in Nutrition from an accredited college/university is required.
Hospital Dietation experience preferred
