Job Title: Social Services Worker
Schedule: Day
Pay Rate: $20.00 – $27.00 per hour

Qualifications:
· Experience with Microsoft Word and Excel
· Strong communication and interpersonal skills
· Ability to work independently and as part of a multidisciplinary team
· Excellent organizational and documentation skills
· Commitment to ongoing professional development
Minimum Qualifications:
- High school diploma
Preferred Qualifications:
- Bachelor’s degree in human services or related field
- Experience in skilled nursing facility or long-term care facility
Job Duties:
- Assist the Social Services Supervisor in the planning, developing, organizing, implementing, evaluating, and directing the Social Services programs of the facility
- Coordinate with other departments to assist with discharge planning of patients from the facility, including coordinating with agency services, equipment, agency referrals, and transportation
- Provide information to residents/families as to Medicare, Medicaid, and other financial assistance programs available to the resident
- Review and document in the medical record
- Assist with psychosocial needs of residents, including development and implementation of social care plans and resident assessments
- Assist in obtaining resources from community social, health, and welfare agencies to meet the needs of the resident
- Recognizing significant resident distress or change in condition during care and communicating this information to the appropriate personnel
