Job Title: Nurse Manager
Schedule: Days
Job Duties:
- The Care Manager facilitates patient care by assessing patient needs, evaluating treatment options, creating treatment plans, coordinating care, and gauging progress.
- Facilitates and actively participates in interdisciplinary planning of clinical care of patients and assists with outcome report completion.
- Serves as care liaison between patient, physician, care team members, and discharge planner by coordinating, monitoring, and communicating patient’s progress.
- Initiation and implementation of care plans as necessary through monitoring and re-evaluation to accommodate changes in treatment or progress.
- Communicates discharge information to physician and payer with outcome data, as appropriate.
- Make written and oral reports/recommendations concerning the activities of your shift as required.
- Ensure that all nursing service personnel are in compliance with their respective job descriptions.
- Participate in the development, maintenance, and implementation of the facility’s quality assurance program for the nursing service department.
- Participate in facility surveys (inspections) made by authorized government agencies as may be requested.
- Conduct accident/incident investigations and give completed reports to Director of Nursing.
- Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting
- Other Duties as Assigned
Minimum Qualifications:
- RN or LPN License is required.
- Care Manager experience is preferred.
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
- Demonstrated leadership abilities.
- Must be knowledgeable of nursing and medical practices and procedures.
