Department: CC018011 MS Cancer Center Support Services Job Description Summary: Oncology Navigator Certified Generalist certification to be obtained after 1 year from hire Position Responsibilities Identifies any barriers to care and refers patient/family to nutrition services, social services, integrative medicine, or psycho-oncology appropriately and in a timely manner. Maintains documentation of all patient encounters and complete reporting requirements according the organization standards. Flexible and adaptable in response to changing patient and healthcare providers' needs. Attends and represents the organization at a professional conferences, in-service trainings, and meetings at the request of or with the approval of the supervisor. Regularly participates in disease specific and general tumor boards and the monthly interdisciplinary meetings (IPCC). Supports the patient/family on thir cancer journey from pretreatment education, active treatment, survivorship and end-of-life care. Collects, analyzes and reports clinical navigator outcome data, metrics, and productivity benchmarks, working with administration, IT, and the cancer registry staff as appropriate. Provides patient education to assist with informed decision making, improve timeliness of care, and multidisciplinary care processes. Works with physicians/APPs and other members of the healthcare team for pathology, and molecular laboratory testing requests and results. Facilitates timely scheduling of appointments, diagnostic testing, and procedures to expedite the plan of care and to promote continuity of care. Qualifications License/Registration/Certification None Education Bachelors degree in related discipline preferred Experience 3 - 5 years experience in patient/healthcare relations or human services field (e.g. nursing, teaching, counseling), preferred, or equivalent combination of education and experience, preferred.
