MSA Primary Care is an independently owned Primary Care Practice in Newark Ohio. We are seeking a diligent, dependable, and hardworking Patient Care Coordinator to join our team! The ideal candidate would be, dedicated, compassionate, have a good work ethic, and excellent customer service skills. This individual will be comfortable working in a fast-paced environment and a large private practice with over six providers.
Job Duties Include:
- Assess and screen patients as well as providing them with education about the medical processes, procedures, health, and wellness.
- Assisting the care teams with goal setting for quality assurance and best practices.
- Following up with patients after being discharged from the hospital and ER visits, reconcile discharge instructions and medications, making sure home health is in place if indicated, assisting with continuing care, scheduling hospital/ER follow up appointments.
- Care management and care planning for identified patients.
- Acts a liaison between physicians and patients.
- Assists in monitoring blood pressure and blood sugars on high risk patients.
- Monitor and track ECQM's.
- QA checks to make sure ECQM's are tracking accurately.
- Encounter reviews to check for failing measures or to assess for a change in risk status.
- Update information as needed for resource stations in lobbies.
- Collaborate with payers for care management and close care gaps.
- Identify social needs and connect or provide information to a community resource.
- Stay up to date on MIPS activities, information, and requirements.
- EHR administration - creating and managing functions within the EHR to ensure proper tracking and recording of ECQM's.
- Fill in when needed on the clinical floor.
