What are the responsibilities and job description for the Medical Records Representative position at MRO?
MRO Corporation, a market leader of disclosure management solutions for healthcare organizations, has an opening for a person to work in the healthcare sector supporting disclosure management services.
Healthcare organizations must have a health record that is created and maintained to meet the requirements of a legal business record. It must be maintained in a manner consistent with state and federal requirements. Health Information Management (HIM) professionals are charged with reconciling health records, as well as certifying them as accurate and complete. Proper management of corrections made within the health record is imperative to maintaining the best quality and integrity of information possible for patient safety.
ESSENTIAL DUTIES AND RESPONSIBILITIES-Overall Skills and Knowledge:
Medical Records Chart Analysis and Quality
Ÿ Processes all requests (received via telephone, email, fax, My Chart/ work queues or inbox) for review and correct data integrity problems with ePHI in the electronic medical record. These changes include, but are not limited to:
o Correction of Survivor MRN”s - Merging/Unmerging of patient (Medical Record Numbers).
o Correction of Demographics information (i.e. name, address etc.).
o Performs daily chart analysis for incomplete documentation of patient records.
Notifies appropriate physicians of delinquent records as needed.
Ÿ Facilitate emergent wrong registrations through the Contact Move process on inpatients and emergency department patients, interfacing with and directing multiple stakeholders as to the appropriate actions to take to quickly resolve data quality issues as a result of wrong patients being registered in a manner and timeframe to minimize the impact to patient safety.
Ÿ Work closely with Patient Registration, and Information Services to solve issues arising from inaccurate patient data or duplicates or other inaccuracies in the electronic patient database.
Ÿ Be a resource for physicians and support with HIM electronic documentation issues.
Ÿ Performs Provider clearances upon request (termination/resignation of employment).
Ÿ Assist Revenue Team (Inpatient/Outpatient coders) with chart completion (follow up calls to providers).
Release of Information
Ÿ Logs requests for medical records received in HIM using ROI tracking software
Ÿ Processes requests in a timely manner and following applicable regulations, rules, and law
Ÿ Assists customers with medical record requests at the Storefront offices.
Ÿ Maintains thorough knowledge of HFHS “Release of Information” policies and procedures guide and uses judgment in releasing information.
Ÿ Prioritize and process requests for continuity of care
Ÿ Answers phone calls from customers regarding release of information status and questions
Ÿ Processes all Transition of Care/ Care Everywhere requests.
Ÿ Assists with the training for new hire Release of Information Specialists
Chart Processing:
Ÿ Pick up discharged patient charts from units
Ÿ Assembly of records according to HIM department procedures
Ÿ Indexing correct document types ensuring components upload to correct area of the electronic medical record
Ÿ Prep, scan, index and QA according to OnBase procedure and policies
Ÿ Performs other related duties as assigned
Ÿ Performs retrospective chart audits for scanning quality.
Ÿ Assist with clinic education and follow up resulting from audits.
EDUCATION
Ÿ Registered Health Information Technician (RHIT) or eligible, preferred
MINIMUM EXPERIENCE
Ÿ One to three years of prior experience in a Health Information Management/Medical Record Department, preferred.
Ÿ Experience with Microsoft Office products (word, excel,)
Ÿ Experience with computers, electronic medical record and release of information software, preferred.
Ÿ Experience with scanning and indexing programs, preferred.

WORKING CONDITIONS:
Normal office environment with minimal exposure to noise, dust, or extreme temperatures.
Remote work could be an option depending on work schedule.
Must be able to lift 25-30 pounds (paper records, transportation buckets, etc.)
