Role Description
This is an on-site, full-time position for a Physical Therapist located in Franklin, TN. The Physical Therapist will be responsible for assessing patient conditions, developing and implementing personalized rehabilitation plans, and collaborating with healthcare professionals to ensure optimal care. Additional duties include monitoring patient progress, educating patients and families on self-care techniques, and maintaining accurate patient records. The role requires a strong commitment to enhancing patient outcomes and fostering a supportive environment for rehabilitation.
Qualifications
- Expertise in Physical Therapy and Rehabilitation techniques
- Strong foundation in Medicine and Patient Care practices
- Experience in Home Care and providing personalized treatment plans
- Ability to collaborate effectively within a multi-disciplinary healthcare team
- Strong communication, documentation, and patient education skills
- Doctorate in Physical Therapy from an accredited program
- Active state license to practice Physical Therapy in Tennessee
- CPR certification and knowledge of best safety practices
