Position Summary
The Registered Nurse (RN) – Home Health Admissions is responsible for completing start-of-care (SOC) and admission visits in patients' homes. This role conducts comprehensive in-home assessments, completes required documentation (including OASIS, as applicable), and collaborates with the interdisciplinary team to ensure safe, compliant, and patient-centered home health services.
Key Responsibilities

Complete home health admission and start-of-care visits in patients' homes.

Perform comprehensive nursing assessments, including physical, psychosocial, and environmental evaluations.

Accurately complete OASIS assessments and all required admission documentation in compliance with CMS and agency standards.

Collaborate with physicians, therapists, and interdisciplinary team members to develop individualized plans of care.

Educate patients and caregivers on diagnoses, medications, treatment plans, safety, and disease management.

Administer medications and treatments as ordered and within scope of practice.

Monitor patient status and promptly communicate changes in condition to the physician and care team.

Coordinate services, referrals, and follow-up visits to ensure continuity of care.

Adhere to infection control, safety, and documentation standards in the home setting.
Qualifications

Active Registered Nurse (RN) license in the State of Connecticut.

Bachelor of Science in Nursing (BSN) preferred.

Minimum of 1 year of nursing experience; home health, admissions, or start-of-care experience preferred.

Current Connecticut driver's license with reliable transportation and ability to travel within assigned service area.

Knowledge of OASIS-E documentation and CMS home health regulations strongly preferred.

Familiarity with Connecticut Department of Public Health (DPH) regulations for home health agencies.

Strong clinical assessment, critical thinking, and time-management skills.

Excellent communication and interpersonal skills with patients, families, physicians, and interdisciplinary team members.

Ability to work independently in the field while collaborating with a multidisciplinary team.

Proficiency with electronic medical records (EMR) and mobile documentation systems.

Salary Range: $79,000 - $93,000 annually (commensurate with experience and qualifications) per Connecticut pay transparency law.
