Responsibilities:
- Provide pharmaceutical care to patients by dispensing medications and providing medication therapy management
- Collaborate with healthcare professionals to optimize patient outcomes and ensure appropriate medication use
- Perform medication order verification and review for accuracy and appropriateness
- Conduct patient consultations and counseling on medication use, side effects, and potential drug interactions
- Monitor patient response to therapy and adjust medication regimens as needed
- Participate in medication reconciliation processes to ensure continuity of care
- Provide education and guidance to pharmacy staff and other healthcare professionals on medication-related topics
Skills:
- Strong knowledge of pharmacy practices, regulations, and standards
- Proficiency in utilization management and medical management principles
- Experience in pediatrics, pain management, and patient care settings
- Ability to provide exceptional patient service and demonstrate empathy and compassion
- Excellent communication skills to effectively interact with patients, healthcare professionals, and pharmacy staff
- Detail-oriented with strong organizational and multitasking abilities
- Pharmacy residency or advanced training in a specialized area is a plus
