Company Description Mountaineer Family Medicine is a multi-specialty provider serving patients across Kanawha City, Nitro, Chapmanville, Oceana, and Welch, West Virginia. The practice offers a broad range of services, including primary care, addiction services, and diet treatments, supporting patients across diverse health needs. Additional services include joint injections, lab services, Botox injections, and tattoo removal. The team focuses on accessible, community-based care and comprehensive support for individuals and families.
Role Description This is a full-time, on-site Nurse Practitioner role based in Fairmont, WV. The Nurse Practitioner will provide direct patient care, including comprehensive assessments, diagnosis, and treatment planning for a variety of acute and chronic conditions. Daily responsibilities include conducting patient exams, ordering and interpreting diagnostic tests, prescribing medications, and performing procedures within scope of practice. The role involves coordinating care with physicians and other clinicians, documenting encounters in the electronic health record, and supporting preventive care and patient education. The Nurse Practitioner will also collaborate with the care team to manage chronic disease, support addiction treatment plans where applicable, and contribute to a positive, patient-centered environment.
Qualifications
- Current, unrestricted Nurse Practitioner license in the state of West Virginia and national certification (e.g., FNP, ANP, or relevant specialty).
- Demonstrated clinical competence in primary care, including assessment, diagnosis, and management of acute and chronic conditions.
- Strong skills in patient communication, education, and counseling, with a focus on compassionate, culturally sensitive care.
- Ability to work collaboratively within a multidisciplinary team and coordinate care across services.
- Proficiency with electronic health records and accurate, timely clinical documentation.
- Experience or strong interest in addiction services, preventive care, and community-based medicine is preferred.
- Master’s degree in Nursing or higher from an accredited program; BLS/ACLS certification required or ability to obtain.
- Strong organizational skills, sound clinical judgment, and the ability to manage a steady patient caseload in a fast-paced setting.
Office hours are Monday through Thursday 7:30AM to 5:00PM and Friday 7:30AM to 12:00PM. No evenings, weekends or holidays. Paid time off and paid holidays.
