The Clinical Case Manager serves as an integral member of the clinical team, providing direct services and supportive functions to the patients at Mountain Youth Academy. Responsibilities include conducting assessments, facilitating groups, representing MYA at team meetings and court hearings, reviewing documentation for accuracy and timeliness, and discharge planning.
Must be familiar with and understand all payor source regulations as they relate to Psychiatric Residential Services; regulation promulgated by CMS as they relate to patient care, treatment and services. Performs weekly audits for charts concurrently to ensure that the medical records documentation meet the requirements of the regulatory of all regulatory bodies.
Documentation Skills, Analyzing Information, Decision Making, Research Skills, Verbal Communication, Written Communication, People Skills, Conflict Resolution, Initiative, People Management, Integrity, Connecting with Children, proficiency in Microsoft office systems, able to develop professional relationships with DCS workers as well as court representatives, and able to recognize when a patient is becoming upset during visitation, court, or potentially distressing events.
Additional Responsibilities
Must have a bachelor’s degree (in humanities, social work, or related field preferred). Previous experience in medical records is ideal. Must be a self-starter with the ability to communicate effectively with other health care professionals, both verbally and in writing. Exceptional attention to detail needed.
