(FT/PT)Our Mission: We Celebrate and Enhance All Lives with Kindness & IntegrityHighlights of the Memory Care Coordinator Position:The Memory Care Coordinator oversees the overall implementation, delivery and coordination of the Connections Memory Care neighborhood resident services, including engagement and supports the MCD/HWD in non-clinical health services.Coordinates engagement aspects of life for Memory Care residents including participating in the development of personalized plan or program for successful aging in community.Ensures residents satisfaction and achievement of their realistic goals.
