Mountain View Hospital is looking for a Population Health Management Supervisor to join our team!
JOB SUMMARY: Oversees daily operations of the population health management team to ensure smooth operations and to provide patients with the best possible care experience from Mountain View Organization.

Qualifications:Education/Certification: RN degree with BLS certification
Experience: 6 months required experience, 2-5 yrs clinical experience preferred
Equipment/Technology: Must be able to use Microsoft Office, WORD, Powerpoint, Excel, Microsoft Office, and various EMR systems.
Language/Communication: Must have the sensitivity, maturity, and ability to communicate clearly and concisely, both verbally and in writing.
Mathematical Skills: General mathematics.
Mental Capabilities: Can concentrate for long periods of time in order to complete assigned tasks with a better than average attention span in order to listen, read and remember verbal and written instructions. Can understand and act upon input from the manager concerning errors and instructions.
Interpersonal: Ability to instruct and work well with others as part of a team.
Performance: Full time position. Ability to assist in conflict resolution with both patients and staff members. Must be able to pay attention to detail while having a focus on deadlines.
