Description

Mountain View Hospital is looking for a Population Health Medical Assistant to join our team!

JOB SUMMARY:       

The Population Health Medical Assistant supports MVH’s initiatives toward clinical integration and accountable care strategies to achieve the triple aim in healthcare: 1) improving quality and patient satisfaction, 2) improving outcomes and the overall health of the population, and 3) reducing the total cost of care. Through reporting tools available to Population Health, the Population Health MA identifies gaps in care for patient’s and coordinates care for the patient’s with PCPs and other care providers. The Population Health MA collaborates with clinical teams as appropriate for further intervention and direction. The Population Health MA will have a strong clinical background and will provide patient care during wellness fairs and community events.

DUTIES AND RESPONSIBILITIES:

  1. Engages with population health team on Value Based Care Models and has a solid understanding of the various programs we are engaged in and our responsibility related to them
  2. Assist nursing staff in preparing and presenting education to clinic staff
  3. Will participate in webinars, conferences, educational meetings (virtual and in-person) as scheduled by management team
  4. Consults other departments as appropriate for an interdisciplinary approach to the patient’s needs.
  5. Maintains a good working relationship both within the department and with other departments within our organization. Communicates appropriately and clearly to manager, co-workers and physicians.
  6. Reports to work on time and as scheduled, completes work within designated time
  7. Complies with all organizational policies regarding ethical business practices and safety for patients and staff. Maintains patient confidentiality at all times.
  8. Maintain flexibility in a growing environment and be willing to grow, change and accept new duties as the scope of our department changes with the needs of the community and hospital. This includes flexibility with work schedule to work weekends, Holidays, and overtime when needed
  9. Collection of appropriate blood samples while ensuring proper patient identification and labeling is completed
  10. Administration of adult and childhood immunizations.
  11. Ensures patients receive competent and quality clinical care. This includes but is not limited to:
    1. Adherence to all policy and procedures put in place by organization and department
    2. Provides age appropriate care
    3. Reporting, monitoring, and management of patient and staff incidents
    4. Monitoring and management of patient and physician satisfaction

Qualifications


Education/Certification:  Medical Assisting or equivalent. Phlebotomy certification is a plus.

Experience: 6 months required experience in phlebotomy, 2 years clinic experience preferred 

Equipment/Technology:  Experience with Word, PowerPoint, Excel, Microsoft Office, and various EMR systems. Experience with scanning, faxing, and electronic medical record organization is a must

Language/Communication: Ability to read and interpret documents such as quality measure implementation guidelines and requirements as well as utilization reports.  Ability to speak effectively before groups or employees of the organization. Must be patient, courteous, compassionate, and respectful with patients and their families, co workers, physicians and administrative staff.  Ability to communicate medical terminology and relate well with physicians, staff and management

Mathematical Skills: General mathematic skills

Mental Capabilities: Can concentrate for long periods of time in order to complete assigned tasks with a better than average attention span in order to listen, read and remember verbal and written instructions.  Can understand input from the supervisor and/or manager concerning errors and instructions. Ability to deal with problems involving several concrete variables in standardized situations.  A clear understating of the importance of patient confidentiality and sensitivity regarding complex health problems. Ability to use critical thinking skills to assess patients and intervene as needed  within scope of practice  

Interpersonal: Ability to work well with others as a team player. Organized, detail oriented, and the ability to manage to multi-task while maintaining a heavy load of complex patients. Maintain a neat and professional appearance.

Performance:  Must be able to handle conflict issues with both patients and staff. Must follow established recommendations and guidelines. Must meet deadlines as needed.
