Mountain View Hospital is looking for a Social Worker Discharge Planner to join our team!
JOB SUMMARY:
A hospital social worker/discharge planner supports patients and families by coordinating safe discharge plans, connecting them with community resources, and addressing social, emotional, and practical needs during hospitalization.
Key Responsibilities
Patient assessment Evaluate patients' social, emotional, financial, and support needs to identify barriers to discharge.
Discharge planning Develop individualized discharge plans, arrange follow‑up care, and coordinate services such as home health, rehabilitation, or long‑term care.
Resource coordination Connect patients and families with community agencies, financial assistance, transportation, and support programs.
Interdisciplinary collaboration Work closely with nurses, physicians, case managers, and therapists to ensure smooth transitions of care.
Patient advocacy Ensure patient rights are upheld and help families navigate complex healthcare systems.
Documentation Maintain accurate, timely records of assessments, plans, and interventions.

Qualifications
Education/Certification: Must be a Licensed Social Worker with a Bachelor's or Master's degree in Social Work.
Experience: Knowledge of medical terminology, hospital workflows, and community resources, preferred.
