Virginia Locations: Galax, Martinsville

Responsibilities:
- Provide skilled nursing care to hospice patients in accordance with the plan of care.
- Assess patients’ physical, emotional, and social needs.
- Monitor patient conditions and make necessary adjustments to care plans.
- Provide education and emotional support to patients and their families.
- Collaborate with interdisciplinary teams to deliver holistic care.
- M – F
- Day Shift: 8a – 5p
- Every 7th weekend
- Graduate of an accredited nursing program.
- Current RN licensure.
- Valid Driver’s License
