Are you a compassionate and dedicated social worker looking to make a real difference in the lives of patients and families facing end-of-life challenges? At Mountain Valley Hospice, we provide exceptional care and support, and we’re seeking a PRN Hospice Social Worker to join our team. This role is more than a job—it’s an opportunity to bring comfort, guidance, and advocacy to those who need it most.
What You’ll Do:
✔ Support & Advocate – Assess patients and families, develop personalized care plans, and provide emotional and practical support to enhance coping skills.
✔ Educate & Guide – Help families navigate the dying process, anticipatory grief, and bereavement while ensuring they have the resources they need.
✔ Collaborate & Communicate – Work closely with an interdisciplinary team, including nurses, chaplains, and volunteers, to advocate for patient needs.
✔ Connect to Resources – Make referrals to community organizations, chaplains, and volunteer programs to provide additional support.
✔ Stay Organized – Maintain timely and accurate documentation to ensure seamless care for each patient.
What We’re Looking For:
✔ A bachelor’s or master’s degree in social work, sociology, or psychology from an accredited institution.
✔ At least one year of social work experience in a healthcare setting.
✔ A compassionate professional who is comfortable traveling to patients’ homes, skilled nursing facilities (SNFs), assisted living facilities (ALFs), and hospitals to provide care.
