Summary
We are seeking a VCAP Program Advisor located in our community centers. This role is vital in supporting individuals with disabilities by providing guidance, mentorship, and resources that align with our mission of fostering independence and self-advocacy.
Essential functions:
1. Ability to travel.
2. Ability to occasionally work long hours and/or weekends.
Responsibilities
1. Service Provision
a. Receive Veterans Community Access Program (VCAP) referrals, including Veteran assessment information and an assigned annual allotment, from MTSTCIL/FIL Program Manager
b. Within two business days of referral, the VCAP Advisor will contact the Veteran by telephone to schedule an in-home visit within seven business days whenever possible, to provide information about the Veterans Community Access Program (VCAP), and to facilitate the Veteran in making an informed decision to participate or to decline to participate.
c. Review the Veteran's support needs with the Veteran and assist him or her in the development of a Spending Plan
d. The VCAP Advisor shall provide information.
e. Report changes in the Veteran's condition and/or needs.
f. Recognize and report critical events, abuse, or neglect.
g. Request a review of decisions, reporting grievances and dispute resolution.
h. Communicate face-to-face and by phone with Veterans/ARs and, when requested by the Veteran, with others such as family members and/or caregivers.
i. Remain in contact and be available to the Veteran as needed to accomplish the services of this Contract, and shall make and document not less than one contact during each calendar month of the Veteran's enrollment in the Veterans Community Access Program (VCAP), and one face-to-face visit with the Veteran/AR quarterly.
j. Advise Veterans.
k. Provide support to all Veterans referred by MTSTCIL/FIL or its designee
l. Transmit to appropriate parties, within a timely manner: the signed enrollment forms and packets, assessments, Spending Plan or other necessary documents
m. Assure each Spending Plan submitted can be carried out within the Veteran's assigned allotment and that it includes payments only for services and goods allowable and approved by Veterans Community Access Program (VCAP) policies
n. Assist the Veteran to submit monthly invoices with units of service itemized by Veteran within Ten calendar days after the last day of each month if needed.
o. Transmit in writing to MTSTCIL/FIL, within three business days, whenever a Veteran's residence status changes or a Veteran transition out of the program to include the reason for the change/transition.
p. Develop and implement program policies and procedures congruent with the philosophy of Veterans Community Access Program (VCAP) and consistent with Veterans Community Access Program (VCAP) program policies.
q. Exchange data electronically with MTSTCIL/FIL in congruence with standards of the Health Insurance Portability and Accountability Act (HIPAA)
r. Report any suspected or alleged abuse, neglect or exploitation to APS and MTSTCIL/FIL Program Manager.

Qualifications
1. High School Diploma
2. Experience working with people with disabilities
Required Skills:
1. Demonstrated ability to communicate effectively, orally and in writing, with or without reasonable accommodation.
2. Familiarity in using computers for work processing, data collection and communication.
3. Demonstrated ability to conduct research and analyses of pertinent legal authority.
