Full time position for Patient Care Navigator
The Patient Care Navigator is responsible for coordination of dental services and assisting patients with navigating the healthcare delivery system to promote optimal oral health. Their duties include completing intake paperwork and updating patient records, sterilizing and organizing dental tools and monitoring patients for treatment plan completion or barriers preventing treatment plan adherence.
· Completion of social determinant needs with a standardized screening tool.
· Managing dental schedules for optimal workflow
· Responsible for greeting patients as they arrive.
· Submit billing details for dental services and update patient records.
· Coordinate dental referrals for specialty dental services.
