Job Description Summary

Position Responsibilities

  • Overcome barriers within the healthcare system to ensure patients receive the care and treatment needed; assigning patients appropriately to designated navigators, while balancing caseloads.
  • Coordinates with other departments to ensure efficient patient flow and referral process to other team members is initiated and maintained.
  • Supervises daily preparation of new patients for next day visits to ensure all necessary medical records are available or are scanned in the patient's chart, and ready for the physician visit.
  • Participates in the implementation of department initiatives and projects; ensure team is involved in activities to meet objectives/goals.
  • Utilizes various administrative tools (forms and system applications) to track patient intake and treatment interventions and outcomes, summarizing and reporting findings using the navigator dashboard.
  • Handles complex patient situations, assisting the other navigators as needed, while working on assigned caseload. Involves and communicates with the department Director in decision-making process, quality improvement projects and initiatives.
  • Communicates strategies to the team and guiding them towards targets and departmental goals.
  • Works closely with the department Director to ensure established goals are aligned and monitors team compliance, and submitted to the Director on a monthly basis.
  • Builds relationships with other patient navigators, including orientation and training of new staff. Promotes a culture of patient safety and quality care is our top priority.

Qualifications

    • None
  • Education
    • Bachelor's Degree in healthcare or related field preferred, or completed within 2 years of hire.
  • Experience
    • Minimum of 3-5 years of experience in patient/healthcare relations, or equivalent combination of education, experience and training. Training in health and community outreach programs a plus. Some leadership experience a plus.

Follows the completion of incident reporting policy., Core: Understands all new physician orders must be implemented within two hours of initiation for routine and within 30 minutes or less for stat., Core: Understands and is able to administer medication per physicians orders adhering to the hospital approved patient identifiers, following the 5 rights of patient medication administration., Core: Understands and is able to prepare patient for invasive and / or non invasive medical procedures ensuring universal protocol compliance., Core: Understands the procedures to ensure continuity of patient profile/kardex and/or procedure preference cards or other unit specific tools, in a timely manner. Assures continuity of patient care assessment and reassessment as per policy., Core: Utilizes chain of command to communicate changes in patient condition and initiate timely interventions to prevent negative patient outcomes., Job Specific: Builds relationships with other patient navigators, including orientation and training of new staff.
