JOB SUMMARY:

The Qualified Mental Health Professional II (QMHP II-A) Geriatric Transition Center provides support and training to geriatric adults in a short-term unit that provides both residential crisis transition and medically-managed services. Responsible for developing and implementing intervention strategies, as well as leading individual and group support classes to assist in recovery. Provides education, empowerment, and support to the individuals in the unit who are addressing mental health, co-occurring needs. Provides supervision of individuals on a group or individual basis, depending upon unit need. Collect data and provide accurate documentation in accordance with regulatory standards. Provides crisis intervention services as needed.

ESSENTIAL FUNCTIONS:

  • Provide individualized support and training services to adults meeting criteria for the Geriatric Transition Center in order to promote mental, behavioral, and substance use recovery and meet the program goal of reducing reliance on more restrictive inpatient services.
  • Provide ongoing assessment, supervision, and monitoring of individuals at various care levels while in the Geriatric Transition Center, either on a group or individual basis as determined by unit need. Ensure coordination of services with other service providers. Assist individual directly to locate, develop, or obtain needed services, resources, and appropriate public benefits. Provide transportation to and from scheduled appointments, social and recreational activities, shopping trips, and other Agency programs.
  • Collect and maintain data and provide accurate documentation appropriate to the needs of clients served in accordance with all regulatory and Licensure standards while complying with confidentiality regulations.
  • Assist program staff in coordination of service delivery to ensure that individuals receive effective and appropriate treatment; makes referrals and provides advocacy as needed.
  • Assess, evaluate, and monitor the health and safety of the individuals. Initiate and facilitate de-escalation techniques and strategies to assist the individual in maintaining self-control. Monitor individual's health status including any medical conditions, medications, and potential side effects and assist with accessing primary care and other medical services as needed. Provide crisis interventions, as necessary.
  • Provide training to individuals, family members, and other caregivers and service providers in a person-centered approach to assist the individual in living in the least restrictive environment.
  • Maintain required certifications and attend mandatory training.

OTHER DUTIES:

  • Perform such other duties as assigned by supervisor.

QUALIFICATIONS:

  • Good oral and written communication skills.
  • Knowledge of mental health, developmental disability, substance abuse, DD symptomology and confidentiality guidelines.
  • Knowledge of treatment modalities and case management approaches, such as service coordination, behavior management, independent living skills training, supportive counseling, family education, and discharge planning.
  • Knowledge of crisis intervention strategies and procedures.

Job Requirements

EXPERIENCE/EDUCATION REQUIRED:

Bachelor's degree required.

A "Qualified mental health professional" means a person who has (i) completed, at a minimum, a bachelor's degree.

After hire, the candidate must complete the required approved training and education pathways for QMHP-T and QMHPs, including registration with the Virginia Department of Health Professions Board of Counseling
