Compensation
Mount Prospect Academy is seeking a Permanency Coordinator (PC) to support youth ages 11–21 in residential care by building and strengthening lifelong family and community connections. This role is critical to helping students transition successfully from residential treatment to long-term stability.
The Permanency Coordinator collaborates closely with families, caregivers, community providers, and internal clinical teams to ensure coordinated care, long-term planning, and positive outcomes for youth with complex behavioral, emotional, and mental health needs.
- Support youth and families in developing sustainable permanency plans and lifelong connections
- Build strong partnerships with families, caregivers, community providers, and referral sources
- Provide family support services, psychoeducation, and evidence-based interventions
- Conduct home, school, and community-based meetings as outlined in each student’s Individual Treatment Plan (ITP)
- Collaborate with Milieu Clinicians, academic staff, and residential teams on case planning
- Complete required documentation, including:
- Weekly service notes
- Monthly progress reports
- Discharge summaries
- Regulatory assessments (e.g., 30-Day Needs Assessment, Casey Life Skills, Career Assessments)
- Participate in court hearings when requested to represent treatment progress and recommendations
- Coordinate and schedule medical, dental, and follow-along services
- Provide transportation for youth to appointments and family-based activities as needed
- Maintain consistent communication with families and referral systems regarding progress and barriers
- Apply trauma-informed practices, including ARC and TCI, to support youth success
- Seek ongoing supervision, mentorship, and professional development opportunities
- Bachelor’s degree in social work, psychology, education, or a related field
- Minimum of 2 years of experience working with youth with emotional or behavioral challenges
- Valid driver’s license; must be 21 years of age or older
- Ability to successfully complete background checks and required pre-employment screenings
- Successful completion of agency training during orientation
