Role Description
This is a full-time, on-site role based in Westerville, OH, for a Case Management Nurse. The Case Management Nurse will be responsible for coordinating patient care, developing individualized care plans, and ensuring efficient utilization of healthcare resources. Daily responsibilities include conducting assessments, managing cases, planning effective patient discharges, collaborating with healthcare teams, and supporting patients and families with transitioning to appropriate levels of care.
Qualifications
- Proficiency in Case Management and Discharge Planning, with the ability to design and implement comprehensive care plans.
- Strong knowledge and experience in Utilization Management, ensuring patients receive appropriate and cost-effective care.
- Expertise in Nursing and Medicine, with a current and valid Registered Nurse (RN) license.
- Effective communication, problem-solving, and critical thinking skills to manage diverse patient needs.
- Familiarity with healthcare regulations, electronic health records (EHR), and insurance policies is advantageous.
- Bachelor’s or associate degree in Nursing (BSN preferred) and prior experience in case management or a related role is highly desirable.
