Mosaic Healthcare is a community based palliative care program. We take care of patients who are not quite hospice eligible or have chosen to pursue aggressive care. Focus of this position will be on providing compassionate care to patients who are suffering from chronic illness and severe disease burden.
Responsibilities:
- Evaluation and treatment of chronic illnesses (CHF, COPD, HTN, DM, Dementia, Chronic Debility, Malnutrition, Chronic Pain, etc.)
- Will need to understand and be able to manage chronic illnesses
- Be comfortable with managing chronic symptoms (Pain, Nausea, Diarrhea, Constipation, Shortness of Breath, edema, etc.) - Training will be provided.
- Will need a valid DEA and be able to prescribe opioids. Be comfortable managing and treatment chronic pain due to terminal condition under direction of a Physician.
- Be responsible for having treatment goal planning conversations with patients and families, discussing end of life, life limiting conditions, advanced care planning, code status discussions, etc.
- Be comfortable working with physician consultants and teams. A lot of our patients will have primary care physicians as well as groups of specialists.
Training will be provided for the right individual.
Employment will include education from palliative care physicians, other palliative care nurse practitioners and other company prepared training.
Malpractice Insurance will be Provided.
Employee will be expected to see a defined case load daily. Work settings include hospital setting, some clinic, and occasional home visits. When seeing facility patients, employee is expected to see a minimum of 20 patients a day.
Will need to share weekend call with Physicians and other Nurse Practitioners. Will round 1 weekend a month at a hospital facility.
This is an 8 to 5 position. Employee will need to be on site during work times to provide care to patients.
