- Provide Direct Patient Care
- Diagnose, treat, and manage acute and chronic medical conditions in a primary care setting.
- Conduct routine physical exams, preventive screenings, and health risk assessments.
- Achieve and Monitor Quality Metrics (HEDIS/Stars)
- Align care delivery with established clinical quality guidelines (e.g., HEDIS, Medicare Stars).
- Ensure documentation and coding accuracy to reflect quality outcomes.
- Drive Patient Satisfaction and Engagement
- Foster strong physician-patient relationships through effective communication and personalized care.
- Address patient concerns promptly and follow up to improve overall satisfaction.
- Participate in Daily Team Huddles and Care Coordination
- Collaborate with interdisciplinary care teams during daily huddles to review high-risk patients and care plans.
- Coordinate with specialists, case managers, and support staff for holistic care delivery.
- Promote Preventive and Population Health Initiatives
- Identify care gaps and proactively engage patients in preventive care, such as immunizations and screenings.
- Use data-driven insights to support population health management strategies.
- Requires a MD or DO.
- Current unrestricted medical licensure in applicable state(s) and board eligibility or board certification.
- DEA license required.
- For Carelon Health business unit, satisfactory completion of a Tuberculosis test is a requirement for this position.
Salary : $232,000 - $282,785
