Key Responsibilities
Diagnose and manage chronic and acute conditions in elderly and medically complex patients.
Respond promptly to changes in condition and coordinate with facility staff to stabilize patients on-site.
Lead goals of care conversations, including advance care planning, code status, and palliative care options when appropriate.
Ensure that care plans align with patient values and preferences.
Contribute to quality improvement and compliance initiatives aligned with value-based care programs.
Collaborate effectively with nurse practitioners, nurses, case managers, and other facility staff to deliver coordinated and comprehensive care.
Provide oversight and support to NPs and participate in case discussions and care planning meetings.

Key Responsibilities
- Perform Rounds at Assigned Nursing Homes:
Diagnose and manage chronic and acute conditions in elderly and medically complex patients.
- Prevent Hospital Readmissions:
Respond promptly to changes in condition and coordinate with facility staff to stabilize patients on-site.
- Educate Patients and Families / Conduct Goals of Care Discussions:
Lead goals of care conversations, including advance care planning, code status, and palliative care options when appropriate.
Ensure that care plans align with patient values and preferences.
- Close Gaps in Care and Ensure Quality Metrics:
Contribute to quality improvement and compliance initiatives aligned with value-based care programs.
- Document Thoroughly and Collaborate with Care Teams:
Collaborate effectively with nurse practitioners, nurses, case managers, and other facility staff to deliver coordinated and comprehensive care.
Provide oversight and support to NPs and participate in case discussions and care planning meetings.
Qualifications
- MD or DO with an active California medical license
- Board-certified/eligible in Internal Medicine, Family Medicine, or Geriatrics
- Experience in post-acute, long-term care, or geriatric medicine preferred
- Proficient in conducting goals of care conversations.
- Familiarity with SNF regulations, Medicare guidelines, and value-based care models
- Strong interpersonal, leadership, and documentation skills
Salary : $232,506 - $290,632
