Position Summary
 
The Hospital Liaison/Case Manager collaborates with the PACE Interdisciplinary Team to support the coordination and continuity of care for program participants. This role facilitates communication between hospitals, care teams, and PACE sites to ensure that participants receive timely, appropriate care throughout the hospitalization and discharge processes. The Liaison ensures the participant's Plan of Care is effectively implemented and helps manage care transitions across various settings.
 
How You’ll Make an Impact
 
  • Evaluate hospital admissions through concurrent and retrospective reviews to determine medical necessity and appropriate levels of care.
  • Act as a clinical liaison between hospitals and PACE program staff to ensure seamless care coordination.
  • Review and assess medical documentation and service requests for accuracy, necessity, and alignment with established guidelines.
  • Exercise professional judgment in collaborating with the Interdisciplinary Team to develop and implement individualized care plans, ensuring alignment with clinical objectives, participant needs, and regulatory requirements.
  • Assist with discharge planning to facilitate appropriate transitions of care and reduce readmissions.
  • Participate in Quality Improvement and Risk Management activities.
  • Track and report care trends or issues to the Regional Director of Clinical Services.
  •  Apply clinical judgment and independent problem-solving to optimize care delivery, drive efficient resource utilization, and enhance participants' overall quality of life in alignment with care objectives.
  • Advocate for minimizing hospital stays when clinically appropriate.
  • Maintain timely and accurate records of participant interactions and care updates.
  • Provide daily status updates to the Regional Director of Clinical Services.
  • Performs all other duties as assigned.
 
 
 
 
 
What You’ll Bring
 
To perform this job successfully, an individual must be able to perform each primary function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
  • Associate's or bachelor's degree in health sciences, Social Work, or a related clinical field preferred.
  • Minimum of 3 years of experience in a clinical, case management, or healthcare coordination role (hospital, long-term care, or similar setting preferred).
  • Experience with utilization management, discharge planning, or care coordination strongly preferred.
  • Certification in Case Management (CCM or similar) preferred but not required.
  • Proficiency in Microsoft Word, Excel, and electronic medical record systems.
 
Certificates, Licenses, Registrations
 
  • CPR certification required.
  • Other clinical certifications (e.g., CNA, CMA, LPN, MSW) are helpful but not required.
