RN Case Manager

About the Job

Morrow County Health District has a full-time opening for a RN Case Manager in our Rural Hospital in Heppner, Oregon.

The RN Case Manager is a pivotal member of the care team at Pioneer Memorial Hospital, responsible for managing and coordination all aspects of the swing bed admission and discharge process.  Acting as the central liaison between patients, families, referral sources, and clinical teams.  This role ensures timely, appropriate, and seamless transitions into and out of the seeing bed program and all inpatient services at Pioneer Memorial Hospital.   The RN Case Manager must have an active Oregon Registered Nurse license and follow all standards and scope of practice for an RN.  The RN Case Manager must have a strong background in care coordination and swing bed programs, with knowledge of Medicare regulations and rural healthcare delivery, must maintain BLS, ACLS, PALS, and TNCC.  Must have excellent interpersonal communication skills to accomplish a variety of work with various groups of people, including nurses, patients and family members, personnel from other hospitals, clinics, healthcare organizations and facilities, co-workers as well as the general public.   A demonstrated understanding and appreciation for diverse cultures is required.  

 Job Details:

• Monday-Friday 7 a.m.-4 p.m.
•  Wages: $117,520-$136,287

Qualifications:

• Registered Nurse with an active Oregon license
• Pre-employment background check and drug/alcohol screen are required
