Responsibilities:
Conduct nutritional assessments and develop personalized dietary plans for residents, taking into account their medical history, dietary needs, and preferences
Collaborate with the healthcare team to provide comprehensive care, including developing and implementing care plans that address patients' nutritional needs
Provide education and support to residents and their families on proper nutrition and meal planning, promoting healthy eating habits and lifestyle changes
Conduct regular rounds to monitor residents' nutritional status and provide feedback to the healthcare team
Develop and maintain accurate and thorough records of patient interactions, assessments, and treatment plans
Assist in the development and implementation of policies and procedures related to nutrition care, as well as participate in quality improvement initiatives to optimize patient outcomes
Participate in committees and other meetings as required, staying up-to-date on the latest industry developments and best practices

Qualifications:
