Key Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans
- Provide skilled nursing care including medication management, pain control, symptom management, and emotional support
- Act as a liaison between patients, families, and the interdisciplinary care team to ensure coordinated care
- Educate patients and families on disease processes, medications, and end-of-life care options
- Monitor patient progress and adjust care plans as needed to ensure comfort and dignity
- Document patient care and progress in compliance with hospice regulations and company policies
- Participate in regular team meetings to discuss patient status and plan of care
- Current and unrestricted RN license in Pennsylvania
- At least 1 year of hospice or palliative care experience preferred
