Job Title: Registered Nurse
Report To: Executive Director
Supervises: CNA's
General Statement of Duties: To ensure that the entire health care of each patient is maintained and monitored on a consistent basis; to train CNA's in all areas needed to ensure adequate health maintenance for the patient.
Qualifications: The nurse consultant must be a graduate of an accredited school of nursing and be licensed in the state of Rhode Island. It is preferred that she/he have two years' experience, strong medical/surgical background. Competencies expected for this position are the following: ability to teach and train CNA's, organizational skills, and familiarity with state and federal regulations affecting persons within the specific residence(s).
Schedule: the directors will determine the number of hours, with consideration for the needs of the patient and the budget of the agency. The nurse must be flexible and available by telephone at home and shall be provided with cell phone for emergency purposes.
Specific Duties: The following list expands upon the General Statement of Duties above and covers three areas:
Client-related:
· To assess the physical, emotional, behavioral, and medications status of each patient both upon admission and as needed thereafter.
· To initiate contacts with appropriate service providers, once needs have been identified, in order to have needs investigated/evaluated through consultation or appointment.
· To ensure that patients are taken to appointments and those patients' files are available to the service provider for reference.
· To write individual nurse's notes as often as warranted, but at least monthly, assessing the ongoing health care needs of each patient.
· To schedule appointments with service providers as patients' conditions warrant.
· To train patients' families in administration of medications or treatments, if appropriate; to inform family of any problems and be available for questions regarding patient's health care.
· To write annual medical summaries of each patient according to the approved format;
· To record patients' vital signs monthly, or more frequently, if needed.
· To schedule annual physicals, dental appointments, hearing examinations as necessary, eye examinations every two years, or as ordered by physicians, and gynecological examinations yearly or as ordered by physicians.
· To conduct Home Visits
· To conduct Admissions and Intakes
Staff-related:
· To provide in-service sessions for CNA's on diet, nutrition, oral hygiene, communicable diseases, "Body Mechanics", detecting illnesses and dysfunction, first aid, use of Kardex, personal hygiene, and particular patients' conditions.
· To Supervise CNA's during visits
· To properly train the CNA's
Systems Management-related:
· To maintain Kardex, medical records, face sheet medication sheets, and nursing care plans.
· To maintain contact with primary care physicians that meets the needs of patients in a satisfactory manner.
· To request and receive feedback from Directors regularly.
· To inform Administrators about status of the patients; to initiate meetings regarding problems, which need resolution.
· To follow all agency policies and procedures.
· To maintain loyalty and confidentiality with regard to the patient, CNA, and agency.
