This role is ideal for nurses looking for flexible, supplemental income while making a meaningful impact in patient care and compliance.
This position primarily supports required client assessments and care coordination in a home setting. Travel between clients is required.

Responsibilities
- Conduct in-home patient assessments and develop care plans
- Monitor patient condition and document findings accurately
- Ensure compliance with state requirements and assessment timelines
- Communicate changes in patient condition to appropriate parties
- Coordinate care with physicians and other healthcare professionals
- Provide education to patients and families on care plans and conditions
- Maintain accurate and timely documentation
Qualifications
- Active RN license in the State of Maryland
- Home care or home health experience preferred
- Ability to work independently and manage schedule efficiently

- Reliable transportation and willingness to travel between cases
