Job Summary
We are seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) to serve in a dual Administrative and Skilled Nursing role within our home health care agency. This position supports both clinical operations and direct patient care, ensuring high-quality services, regulatory compliance, and excellent patient outcomes.
Key Responsibilities
Administrative Duties:
- Assist with patient intake, start-of-care coordination, and scheduling
- Review and maintain accurate patient records and clinical documentation
- Ensure compliance with state, federal, and agency regulations
- Communicate with physicians, RNs, HHAs, patients, and family members
- Support care coordination, authorizations, and follow-up documentation
- Assist with audits, recertifications, and quality assurance activities
Skilled Nursing Duties:
- Perform skilled nursing visits under RN supervision (as permitted by state regulations)
- Monitor vital signs and patient condition; report changes to RN/physician
- Administer medications and treatments as ordered
- Provide wound care, catheter care, and other skilled services
- Educate patients and caregivers on care plans and disease management
- Ensure patient safety and infection control in the home setting
Qualifications:
- Current LPN license in good standing in the state of employment
- Minimum 1–2 years of nursing experience (home health preferred)
- Strong knowledge of home health regulations and documentation standards
- Excellent organizational, communication, and time-management skills
- Proficiency with EMR systems and basic computer skills
- Valid driver's license and reliable transportation
- Ability to work independently and as part of a team
Preferred Qualifications:
- Home health or community-based care experience
- Administrative or office experience in a healthcare setting
Familiarity with start-of-care, resumption of care, and recertification
