ACTIVITY DIRECTOR

MONUMENT HILL NURSING AND REHABILITATION CENTER - LA GRANGE, TX

Wellsential Health is seeking a vibrant and organized Activity Director to plan and lead fun, engaging, and therapeutic programming for residents in our skilled nursing and long-term care community. What You’ll Do:

• Design and implement individual and group activity programs tailored to residents’ interests and cognitive levels
• Ensure a mix of recreational, sensory, physical, and social events for all levels of functioning
• Complete and maintain all required documentation, including MDS Section E and activity assessments
• Develop and update the activity section of resident care plans
• Lead 1-on-1 in-room activities for residents unable to join groups
• Create and post a monthly calendar of events, plus distribute to residents and display prominently
• Publish a monthly facility newsletter for residents and families
• Coordinate at least two off-site activities monthly (transportation, permission, planning)
• Collaborate on community events and represent the facility at outreach opportunities
• Work with the Corporate Activities Team to plan staff and resident-centered events
• Recruit, train, and oversee the volunteer program
• Attend QA meetings and provide monthly reports related to activity programming

What You Bring:

• Bachelor’s degree in Therapeutic Recreation or related field (preferred)
• Certified Therapeutic Recreation Specialist (CTRS) or eligibility (required for some states)
• Certified Activity Director (Required)
• Minimum 2 years of experience in a social or recreational program, preferably in a skilled nursing or senior care setting
• Experience working with residents with dementia and other cognitive impairments
• Strong organizational, interpersonal, and leadership skills
• Creative spirit with a passion for improving lives through purposeful engagement
