Purpose Statement
Plan, direct, define and coordinate individualized and group therapeutic support and services to patient’s with mental health, behavioral health and substance abuse issues in an outatient setting.
Responsibilities
Experience: One-three years of experience or related experience, preferably in community mental health.
Licenses, Certifications, registrations: Current State of Illinois LMSW (Licensed Master's Social Work) or LPC (Licensed Professional Counselor).

Responsibilities
- Provides a broad range of therapeutic and psychiatric social work services to mentally or emotionally disturbed patients.
- Acts as a liaison with hospitals, clinics, referral sources, and other medical professionals/centers affiliated with the facility.
- Collaborates with psychiatric and interdisciplinary teams in diagnoses and treatment plans.
- Assess relative, cognitive, behavioral or emotional problems.
- Other Duties as assigned
- Perform other functions and tasks as assigned.
