Your Role as the Health & Wellness Coordinator
As the Health & Wellness Coordinator, you’ll play a vital role in supporting resident care and clinical operations within our assisted living and memory care community. Working closely with the Executive Director and Health & Wellness Director, you’ll help coordinate assessments, care plans, medication processes, activities, and dementia-focused programming while ensuring compliance with state, federal, and Americare standards.
This role is ideal for a compassionate, organized healthcare professional who thrives in a small, home-like environment, enjoys collaborating with caregivers and leadership, and is passionate about enhancing the daily lives of residents.
This role may also be known as:
Assisted Living Wellness Coordinator, Memory Care Care Coordinator, Resident Care Coordinator, Clinical Support Coordinator, or Health Services Coordinator.
What You’ll Do
- Assist with resident assessments, care plans, pre-admission assessments, and family conferences as directed by the Health & Wellness Director.
- Support day-to-day clinical and wellness operations for residents in assisted living and memory care.
- Coordinate and oversee medical and recreational services, ensuring a high caliber of resident care.
- Facilitate and organize individual and group activities designed to stimulate residents and support cognitive and emotional well-being.
- Work one-on-one with residents to identify interests, needs, and appropriate wellness programming.
- Coordinate and track dementia training in accordance with DHSS and regulatory requirements.
- Organize and maintain Medication Administration Records (MARs) that have been reviewed and approved by nursing leadership.
- Assist with coordinating physician appointments, ensuring residents are prepared and all required documentation is completed.
- Participate in planning and supporting internal and external community events.
- Take staffing call as needed based on community needs.
- Maintain HIPAA compliance and resident confidentiality at all times.
- Collaborate closely with other departments to support a team-oriented, resident-focused environment.
What You Bring
- Certification / Licensure: Must be a licensed nurse or have completed a Certified Medication Technician or Med Level One course with current state certification.
- Experience: Previous experience in senior living, memory care, or healthcare support roles preferred.
- Skills: Strong organizational skills, attention to detail, and the ability to plan, coordinate, and prioritize multiple responsibilities.
- Communication: Excellent interpersonal and communication skills to work effectively with residents, families, caregivers, and leadership.
- Commitment: A genuine passion for senior care and alignment with Americare’s mission, values, and culture.
- Additional Requirements: Valid driver’s license and ability to meet company requirements related to resident transportation.
Physical Demands
- Stand, sit, walk, bend, reach, crouch, kneel, and change positions frequently.
- Lift up to 25 lbs. and assist with transfers as required.
- Push/pull up to 100 lbs. and maneuver resident wheelchairs (up to 350 lbs.).
- Assist in lowering residents to the floor during falls using proper procedures.
- Utilize required safety equipment, including gait belts, mechanical lifts, and PPE.
- Perform tasks requiring good vision, hearing, and verbal communication.
