Associates are eligible to apply for a lateral transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Part-time Associates who have completed their probationary period can move to full-time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. Probationary period is three (3) months for full-time employees, and four (4) months for part-time employees who works more than 3/5 of the normal workweek. Probationary period will not apply to shift changes.

Montefiore Medical Center is proud to announce the upcoming launch of a new acute inpatient psychiatric facility dedicated to providing high-quality, short-term psychiatric care for children and adolescents. This specialized unit will enhance access to timely, developmentally appropriate mental health services, offering a safe, structured, and compassionate environment to support patients in crisis and help stabilize their condition.

As part of our efforts to build a strong, multidisciplinary team, we are currently seeking a Social Worker. As an integral member of the interdisciplinary treatment team, the Inpatient Social Worker provides a comprehensive range of direct and indirect services to patients and their families. All responsibilities are executed in alignment with Montefiore’s Mission and Values, as well as the standards set by the Department of Social Services and the ethical guidelines of the National Association of Social Workers (NASW).

Responsibilities

• Conduct comprehensive psychosocial assessments for all assigned patients, including history, strengths, risk factors, and social determinants of health.
• Provide short-term therapeutic interventions including supportive counseling, crisis intervention, psychoeducation, individual therapy, family therapy, and group treatment.
• Participate actively in treatment planning with the interdisciplinary team, contributing clinical insights and advocating for patient needs.
• Develop and implement individualized discharge plans in collaboration with the patient, family, and care team.
• Coordinate referrals and linkages to community services including outpatient mental health providers, housing, substance use treatment, higher levels of care and social support services.
• Communicate regularly with outside agencies and post-discharge providers to ensure continuity of care.
• Performs and documents weekly assessments throughout patient's stay.
• Serve as a liaison between the patient, family, treatment team, and external agencies.
• Facilitate family meetings and treatment conferences as needed.
• Collaborate with legal, protective, and government agencies in cases involving guardianship, child/adult protective services, or court-mandated treatment.
• Participate in departmental meetings, case conferences, and ongoing professional development.
• Adhere to ethical and professional standards in accordance with NASW guidelines and hospital policies.
  
  

Requirements

• Master’s Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education required.
• LMSW (Licensed Masters Social Worker) or LCSW (Licensed Clinical Social Worker) or limited permit required.
• Understanding of developmental stages from infancy through elder years, family systems theory, and how various illnesses impact patient/family system.
