Responsibilities
- Conducts a comprehensive assessment, including physical, emotional, and mental status for clients of all ages- infant through adult
- Develops plans of care with realistic outcomes, based on interpretation of assessment of data and consideration of client's preferences
- Performs designated discipline interventions according to agency policies
- Provides client/SO education on prevention or management of illness, functional disability, related changes in health status, and on medications and treatments
- Evaluates the results of clinical interventions to measure progress toward goals
- Reports all pertinent information about client status and progress to appropriate multidisciplinary health team members
- Plans sequence of home visits with attention to issues of acuity, geographical location, and other pertinent considerations
- Graduate of a program in Physical Therapy approved by the American Physical Therapy Association.
- Current New York State Licensure
- Two (2) years at senior level in field or acute medical hospital
- Two (2) years supervised clinical experience in a medical setting preferred, or equivalent combination of education and experience
- Must have a valid driver’s license and ability to provide own transporting during working hours for the purpose of making home visits
