BASIC RESPONSIBILITIES: The RCFE Care Coordinator reports to the Director of Assisted Living. This person is responsible for supervision of the RCFE Charge Nurses and Care Partners . This person is responsible to oversee all clinical activities and does rounds of the Hearth, Ranch House and Stewart Cottage. This person implements facility policies and procedures and coordinates nursing activities with services provided by ancillary providers including Home Health and Hospice.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
RCFE HEARTH, RANCH HOUSE AND STEWART COTTAGE
· Follows established performance standards and implements facility RCFE policies and procedures, as well as Title 22 and Health and Safety Code regulations that apply to RCFE, licensed by the Department of Social Services.
· Provides guidance to Charge Nurses and Care Partners in the event of an incident or emergency.
· Makes rounds 5 days/week to identify changes of condition, areas in which staff need additional training, and any other concerns.
· Serves as a resource for Charge Nurses and Care Partners in the event of a resident health concern.
· Provide education as needed to ensure the highest level of care.
· Identifies Prohibited and Restricted Health Conditions and works with the Director of Assisted Living to ensure that the appropriate notifications are made to the Department of Social Services and required care plans are in place.
· Ensures that Home Health and Hospice care is provided as prescribed and meets the needs of the resident.
· Ensures monthly audits of medication charting and dispensing are conducted.
· Keeps Director of Assisted Living informed of any issues.
· Develops Service/Care Plan upon admission and update annually and/or change of condition.
· Participates in resident care conferences as needed.
· Maintains confidentiality.
· Provides clinical assistance with new admissions to ensure that appropriate documentation, medication, and staff education is completed.

· Attend monthly Leadership Team meetings.
· Attend monthly RCFE Team meetings.
Participate in the weekly Manager On Duty (MOD) rotation with other department heads.
EDUCATION AND TRAINING
· Graduate of an accredited school of nursing
· A minimum of 5 years’ experience
· Current registration in the state of California as a [HD2] Registered Nurse
QUALIFICATIONS:
Excellent verbal and written communication skills. Good computer skills- Word, Excel, and PowerPoint. Presents a neat, well-groomed appearance. Strong organizational skills, attention to detail and timeliness. Able to work independently, determine priorities, and remain calm in the midst of interruptions. Makes resident care/needs a priority and deals with physicians and other outside services in a professional manner. Able to deal graciously with conflicts and maintain confidentiality.
[HD1]Do you want to include the Hearth and RH? Especially since we are "pooling" staff as much as possible.
[HD2]I am not comfortable with an LVN in this position. Having an RN is a HUGE help since neither you or I are an RN.
