Key Responsibilities
- Facilitate Wraparound Team meetings to develop and implement individualized plans that meet the unique needs and strengths of each family.
- Provide training and consultation to community professionals and consumers on the Wraparound process.
- Work collaboratively with the Monroe Wraparound Case Coordination Team (CCT) to establish Child and Family Teams and develop Wraparound Plans and budgets.
- Presents Wraparound Plans & Budgets to the Monroe Wraparound Initiative Case Coordination Team (CCT) for review and approval.
- Supports families and staff in achieving their goals by coordinating additional services as needed.
Qualifications
- Must meet the requirements of a Child Mental Health Professional (CMHP) as defined under the Michigan Public Health Code.
- Bachelor's degree in Social Work or related field required; Master's degree preferred.
- Appropriate Michigan licensure/certification required.
- Experience working within the Wraparound process preferred.
