Description

MonmouthCares (http://www.monmouthcares.org/) is a progressive non-profit agency providing care management for children and youth with emotional, behavioral, and developmental challenges. We are part of NJ’s Children’s System of Care, applying the Wraparound process and using an individualized, strength-based approach to helping families achieve their visions. We are currently seeking a full time Wellness Educator for our Health & Wellness Team (HWT), which is part of our services for children who are dually diagnosed with chronic medical conditions and mental health challenges.

Position Responsibilities

As a Wellness Educator, your responsibility is to engage the youth in seeking, planning, and leading a healthy lifestyle. On a daily basis, you will manage a caseload of your own, and partner with Care Managers to participate in the Wraparound process and the Child Family Teams to provide wellness education and hands-on motivational interventions to children and their families. This is a great opportunity for a wellness practitioner who wants to make a difference in a child’s life.

Specific Responsibilities Of The Wellness Educator Include The Following

  • Participate in the Child Family Teams to help develop self-management health goals to be included in the service plan
  • Obtain medical records for eligible youth from primary care providers, specialists, or school nurse.
  • Provide health education specific to chronic conditions of assigned youth
  • Engage assigned children and families in health promotion planning and activities, including the provision of motivational interventions to increase treatment and medication compliance and support lifestyle changes
  • Facilitate the engagement of youth and families in community support
  • Facilitate regular contact via phone calls and face-to-face meetings with youth and family to monitor progress and provide support for the self-management goals included in the service plan
  • Participate in transitional care and follow-up on assigned youth
  • Adhere to Medicaid standards and regulations governing targeted case management
  • Collaborate with the Family Service Organization (FSO) to initiate caregiver support
  • Partner with Community Relations & Resource Development Team to identify new community resources and barriers
  • Adhere to HIPAA standards and maintain client confidentiality

Requirements

Qualifications:

Knowledge, Skills And Competencies

  • Analytical skills - collect and analyze data in order to determine needs of youth/families serviced and evaluate health-related programs
  • Interpersonal skills - culturally sensitive and responsive to diverse populations; ability to work under supervision as well as collaboratively with the community teams
  • Instructional skills - effective public speaking skills to lead wellness programs, teach diverse groups, and facilitate discussions with youth, families and members of the Child Family Teams
  • Problem-solving skills - creatively and critically think how to improve health outcomes of youth served
  • Writing skills - develop written materials to convey health-related information
  • Computer literate and proficient with MS Office Able to work under supervision and as part of a community team
  • Valid driver’s license; maintain required vehicle insurance according to Agency policy
  • Bilingual skills (Spanish) required

Education

Bachelor's Degree in Health Education or related field or LPN license

Certified Health Education Specialist (CHES) credential preferred

Experience

  • Minimum one year of case management/care management experience
  • Experience in working with at-risk children who have complex needs highly desirable
  • Experience with community relations, resource development, and Wraparound process a plus
