Job Description

Are you a passionate Diabetes Educator looking for a flexible side role that fits around your current commitments?

We’re seeking a knowledgeable and compassionate Diabetes Educator to support individuals in managing their health—without the demands of a full-time schedule.

Monitor My Health is a 501(c)(3) nonprofit with a mission to provide evidence-based health promotion programs for chronic disease prevention and management in the community, and to address the social determinants of health.

Program Schedule & Commitment

  • Start Date: 05/12/2026
  • Session Options (choose one):
  • Tuesdays from 9:00AM or 10:00 AM or 11AM
  • Program Length: 9 weeks
  • Frequency: 1 session per week

Key Responsibilities

  • Education Programs: Group Sessions: Facilitate group education consisting of 9 sessions to create a supportive environment where individuals with diabetes can share experiences and learn from one another. Group sessions will consist of 10-15 participants, and each cohort will meet weekly for 9 weeks.
  • Individual Counseling: Provide one-on-one counseling to patients (up to 20 minutes each), offering personalized guidance on self-management strategies and lifestyle changes.
  • Monitoring and Evaluation: Track patients' progress and outcomes, adjusting educational plans as needed. Evaluate the effectiveness of educational programs.

Experience: Previous experience in diabetes education or a related field is beneficial. Experience working with diverse populations and cultural competence is also desirable.

Qualifications (Required)

  • One of the following credentials:
  • CDCES (Certified Diabetes Care and Education Specialist)
  • Registered Dietitian (RD)
  • Registered Nurse (RN)
  • Bachelor’s or Master’s degree in a relevant field (e.g., nursing, dietetics, health education)
  • Strong knowledge of diabetes management (medication, nutrition, physical activity, psychosocial aspects)
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively within a healthcare team
