Qualifications:
· High School Diploma or equivalent
· Preferred Associates or Bachelors degree in psychology, sociology, or other social services degree or other case management certificate
· Preferred 2 years experience in the behavioral health field
Competencies:
· CLINICAL DOCUMENTATION (Coherent, comprehensive, accurate, & justifies LOC)
· CRISIS INTERVENTION (Demonstrates ability to respond effectively in an emergency; Facilitates a safe,
appropriate intervention in response to client crisis)
· CLIENT DE-ESCALATION (Effectively displays & utilizes non-verbal communication cues i.e. body language)
Responsibilities:
· Serve as a client advocate to support, guide, and coordinate care for clients as they navigate their wellness journey · Provide assistance with clinical and administrative staff
· Lead groups pertaining to case management, self-empowerment and growth if needed
· Enforce structure and regulated client obligations
· Assist with intake assessments as needed
· Shift notes on client behavior, incident reports and warning signs as needed
· Assesses clients, evaluate effectiveness of care plan and progress made by client, participate in client treatment planning and case review
· Perform miscellaneous job-related duties as assigned to better serve client care
· Collaborate closely with Ohana Recovery Residences and Monima Wellness Center staff on client needs and behaviors with encouragement and supportive in pursuing client recovery goals
· Transportation to appointments and client scheduled requirements as needed
· Assist in filing for short term disability through EDD (State of CA specific) or any third-party company that client is associated with if needed
· Assist in filing continued claims for current short term disability claims, including having psychiatrist fill out all required medical paperwork
· Assist in filing for CalFresh if needed
· Provide guidance/review job applications prior to submission and assist in creating or reviewing client resumes
· Assist in setting up aftercare appointments with psychiatrists and therapists for discharge plan
· Provide conflict resolution, safety, and maintain calm redirection in high stress environments
· Strong communication with clients and staff
· Coordinate and encourage engagement with client group attendance and offsite activities
· HIPAA compliance
· Helpful and adaptive with assistance in all staff departments
· Serve as a point of de-escalation for both client and team relations
· Experience with KIPU preferred
· Excellent analytical and problem-solving skills
· Mentor and coach team members to create an environment of growth · Maintain a safe and secure environment for both clients and staff
