Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including long-term care, assisted living, and acute care environments. As an LPN, you will play a crucial role in supporting patients' health and well-being while working collaboratively with other healthcare professionals.

Responsibilities

  • Provide direct patient care, including administering medications and treatments as prescribed.
  • Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
  • Assist with daily living activities, including personal hygiene, feeding, and mobility support.
  • Maintain accurate medical documentation and records in compliance with HIPAA regulations.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Educate patients and their families on health management and disease prevention strategies.
  • Perform procedures such as IV insertion, catheterization, and specimen collection as needed.
  • Participate in discharge planning and coordinate follow-up care for patients transitioning from acute settings.

Experience
Candidates should have experience or familiarity with the following areas:

  • Long-term care, assisted living, or nursing home environments
  • Triage and patient assessment skills in acute care settings
  • Working with individuals with developmental disabilities or dementia-related conditions such as Alzheimer's care
  • Experience in hospice care, geriatrics, pediatrics, or family planning services
  • Proficiency in basic life support (BLS) techniques and infection control practices
