PRN Spiritual Care Counselor/Chaplain

Join our dedicated team as a PRN Spiritual Care Counselor/Chaplain, where you will provide compassionate spiritual support and guidance to patients, families, and staff during critical moments. Your presence will be vital in helping individuals navigate grief, crisis, and emotional distress with empathy and professionalism.

Duties

• Conduct a spiritual assessment as part of the comprehensive assessment within 5 days of admission to the hospice program.
• Assist in development and implementation of the interdisciplinary patient care plan as it pertains to spiritual care.
• Provide spiritual support in accordance with on-going wishes and needs of the patient/family in accordance with their religious preferences or definition of spirituality.
• Work in close collaboration with local clergy when desired by the patient.
• Maintain and submit written clinical records as deemed by the Agency, including the initial evaluation, the care plan and daily notes.
• Lead or participate in religious services, prayer sessions, or spiritual rituals tailored to patient needs and preferences.
• Provide crisis intervention during emergencies or emotional crises, ensuring immediate comfort and appropriate follow-up.
• Facilitate educational workshops, teaching sessions, or presentations on topics related to spirituality, coping strategies, or faith-based practices as needed.
• Collaborate with interdisciplinary teams to integrate spiritual care into overall patient treatment plans.
• Plan and coordinate community outreach events or support groups that promote spiritual well-being and resilience as needed.
• Provide spiritual and bereavement support to staff as needed.
• Provide funeral or memorial services for patient’s as requested.
• Participate in IDG and QAPI meetings and activities.
• Assume responsibility for self-development by continually striving to improve his/her Spiritual practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading.

Experience/Qualifications

• Proven experience in hospice care, hospital chaplaincy, or spiritual counseling with a strong understanding of bereavement support techniques.
• Demonstrated leadership in guiding groups or mentoring colleagues in spiritual or emotional support settings.
• Excellent communication skills for effective public speaking, presentation delivery, and written documentation.
• Skilled in crisis management and crisis intervention strategies to provide immediate support during emergencies.
• Ordained, commissioned or credentialed according to the practices of an organized religious group. OR has a degree in theology or from a seminary.
• Strong event planning abilities for organizing workshops, seminars, or community outreach activities.
• Ability to manage conflicts compassionately while maintaining professionalism and confidentiality.
• Comfortable in an interfaith setting.
• Able to accept different lifestyles, cultures, beliefs and values.

Pay: $26.00 - $33.00 per hour
