Duties
- Respond promptly to after-hours patient calls, assessing symptoms and providing appropriate guidance or interventions
- Coordinate with interdisciplinary teams to develop and modify individualized care plans based on patient needs
- Conduct comprehensive patient assessments, including vital signs and symptom management, to ensure optimal comfort
- Facilitate communication with families, caregivers, and healthcare providers to support ongoing care and discharge planning
- Document all interactions accurately in medical records while maintaining HIPAA compliance and confidentiality
- Manage urgent situations such as symptom crises or emergencies by initiating appropriate interventions or activating emergency services
- Assist with discharge planning and post-acute care arrangements to ensure seamless transitions for patients returning home or to other facilities
Qualifications
- Current RN or LPN licensure with active state certification required
- Knowledge of medical terminology, physiology, and diagnostic evaluation techniques essential for effective patient assessment
- Familiarity with hospice principles, palliative care practices, and post-acute care coordination is advantageous
- Strong communication skills for collaborating with multidisciplinary teams and supporting families during difficult times
- Ability to remain calm under pressure while delivering empathetic patient service in fast-paced environments
