Position Summary
The Health Information Manager (HIM) is responsible for maintaining the integrity of the health information system and medical records for every resident, ensuring they are accurate, systematically organized, complete and readily accessible. The HIM is responsible for the oversight of confidentiality, compliance, privacy and security of the health information system and its contents. Instances where there is only one HI staff, the HIM will complete all necessary HI duties.
Responsibilities
- Health Information Security & HIPAA Compliance Safeguard the confidentiality, integrity, and security of medical records through physical, electronic, and administrative protections in compliance with HIPAA and state and federal regulations.
- Release of Information & Record Retention Manage release-of-information requests and maintain compliant systems for filing, retention, and destruction of paper and electronic medical records per organizational and regulatory standards.
- Census Management & Data Integrity Maintain and communicate accurate daily census data, case mix changes, and pay status updates while ensuring audit trails, current ICD coding, and accurate resident information systems.
- Admission & Ongoing Records Management Coordinate resident-centered admissions, initiate and maintain medical records, perform regular audits and coding, manage physician signatures, monitor orders, and ensure timely and accurate documentation during the resident stay.
- Discharge Record Completion & Quality Review Oversee discharge documentation by assembling, auditing, tracking, coding, and completing medical records while monitoring deficiencies and ensuring timely physician completion.
- Department Leadership, QAPI & Collaboration Supervise Health Information staff as needed, collaborate with leadership on HIPAA compliance and resident rights, support QAPI initiatives, and maintain flexibility to support facility operations.
- Previous experience as a HI Assistant or HUC; training in medical records; experience with coding.
- Knowledge of regulations, survey process, documentation requirements and legal issues.
- Understanding of payment systems including Medicare and Medicaid
- High School graduate or equivalent
- Knowledge of medical terminology
- Long-term care or healthcare experience
