Key Responsibilities:
- Coordinate and oversee individualized care plans for residents.
- Serve as the primary point of contact for residents, families, and healthcare providers.
- Conduct assessments to ensure residents' needs are met and adjust care plans as necessary.
- Advocate for residents' well-being, ensuring their physical, emotional, and social needs are addressed.
- Maintain accurate documentation and comply with all regulatory requirements.
- Collaborate with staff to ensure seamless delivery of services.
Qualifications:
- Bachelor’s degree in social work, Nursing, or a related field (preferred).
- Experience in case management, senior living, or healthcare settings.
- Strong communication, organizational, and problem-solving skills.
- Empathy, patience, and a genuine passion for working with seniors.
