What are the responsibilities and job description for the ACT Case Manager & FSS Worker position at Monadnock Family Services?
Description
This is the entry level professional position in community based psychosocial rehabilitation services for individuals with serious and persistent mental illness. The Case Manager plans, coordinates, provides and monitors therapeutic, rehabilitative, support and community integration services for adults with severe psychiatric disability designed to assist these individuals to integrate as independently and fully into the life of the community as possible. Case Manager works independently and must demonstrate good clinical judgment, decision making, time management and appropriate treatment for consumers in the community.
Requirements
1. Bachelor’s Degree in psychology, social work, psychosocial rehabilitation or related human services field

Associate’s degree in related field and two years’ experience working with individuals with mental illness.
2. Understanding of mental illness and desire to reduce stigma and effectively support this population.
3. Valid driver’s license
4. Required to transport clients as needed
