Grafton City Hospital (GCH) is seeking a compassionate and licensed Behavioral Health Therapist to provide mental health and substance-use disorder services in a supportive, community-based setting.

Key Responsibilities
- Conduct psychosocial assessments and develop individualized, evidence-based treatment plans
- Provide individual, group, and family therapy in outpatient behavioral health settings
- Treat a wide range of issues: depression, anxiety, trauma, substance use, autism spectrum, and more
- Offer crisis intervention, grief support, and coordination of care with other professionals
- Document assessments, treatment plans, and clinical progress in electronic health records (EHR)
- Serve as a liaison to families, community partners, and other hospital departments
- Participate in staff meetings, quality improvement initiatives, and continuing education
Qualifications
Education:
- Master’s Degree in Social Work, Psychology, Counseling, or related field (required)
Licensure (at least one of the following):
- LICSW – Licensed Independent Clinical Social Worker (Preferred)
- LGSW – Licensed Graduate Social Worker
- LCSW – Licensed Clinical Social Worker
- LPC – Licensed Professional Counselor
- PLPC – Provisionally Licensed Professional Counselor
Preferred Experience:
- Mental health counseling and/or substance-use treatment
- Care coordination, case management, or community behavioral health
